at all appointments to ensure proper and efficient billing. We may
verify your address and phone number upon arrival.
New Patients or patient with changes: We request that you arrive 15 minutes ahead of
your appointment time to allow time to complete registration.
Appointments can be scheduled for both the Alexandria Clinic, P.A.
and Osakis Clinic by calling any of the numbers listed below.
Appointments can also be scheduled through any of the
Nursing Stations. If you are
unsure of who you should be seen by, please ask to speak to our triage
nurse.

We ask to be contacted if you will be late for your appointment.
The receptionist will check to see if your provider will be able to see
you at your late arrival time or if your appointment will need to be
rescheduled.
Work related injuries will need a First Report of Injury filed with
your place of employment prior to being seen at our office. For us
to file your claim, we will require the insurance company for your place
of employment and the date of which the injury occurred. If the
workers compensation insurance denies your claim, you will be
responsible for payment of services.
Most appointments can be scheduled within one week. Urgent
needs that require same day attention can be seen by our Express Care.
Physicals may require a longer wait time and may vary depending on the
availability of your provider. Our reception staff will work with
you to schedule a physical as soon as possible. You can also
choose to schedule your physical as you leave your current year's
physical. Reminder letters are sent out for physicals.
If you are unable to be seen by your primary provider, you may be
referred to Express Care. Our Express Care is designed to provide
patients with care on short notices. Express Care is
staffed by specially trained physicians, nurse practitioners, nurses,
lab, x-ray, and receptionist personnel.
All Express Care visits are billed as regular office visits. If
you have insurance, we will automatically bill your insurance company
for your visit.
If you wish to be seen by Express Care, we strongly encourage you to
call ahead. This will allow us to inform you of the best time to
come in based on the number of patients needing to be seen.
Patients will be seen as close to their appointment time as possible.
Walk-in patients may face longer waits as they are worked into the
schedule based on available time and seriousness of their problem.
